Heart Failure After PCI: The Risk That Outlasts the Procedure
Heart failure hospitalization after PCI carries a 25.4% two-year mortality rate — five times the mortality burden of recurrent heart attacks. Standard follow-up wasn't built for this.
Clinical trials have historically grouped post-PCI adverse events — bleeding, recurrent ACS, heart failure — into a single composite endpoint. A 2026 study in JACC: Cardiovascular Interventions, led by Takahiro Suzuki and colleagues at Keio University School of Medicine, broke them apart. The hierarchy it revealed changes how physicians should think about post-PCI survival.
Not all complications are equal
The study tracked more than 10,000 patients across 15 facilities. Heart failure hospitalizations occurred in 4.7% of patients — less common than ACS or major bleeding. But the two-year mortality rate for those patients was 25.4%, nearly three times the rate for ACS and almost three times the rate for major bleeding. In a fully adjusted model, heart failure accounted for 20.1% of total post-PCI mortality — roughly five times the contribution of recurrent heart attacks.
