Dr. Fonseca holds the Tullis–Tulane Alumni Chair in Diabetes and serves as Assistant Dean for Clinical Research at Tulane, where his work has centered on how diabetes care performs outside the trial setting — in the primary care offices where most of it actually happens.

A past President for Science and Medicine of the American Diabetes Association, he edited Diabetes Care from 2007 to 2011, has published more than 500 peer-reviewed papers, and co-developed the BRAVO diabetes risk engine. The ADA awarded him its Banting Medal for Leadership and Service, and Expertscape ranks him among the leading diabetes experts worldwide.

Dr. Shi founded and directs Tulane's Health Systems Analytics Research Center and holds clinical faculty appointments in endocrinology and psychiatry at Tulane's Department of Medicine. His research puts a blunt question to every care model: does it measurably change outcomes, and at what cost?

He has led or co-led more than 40 funded studies for AHRQ, the CDC, NIH, and PCORI, with over 300 peer-reviewed publications. He co-developed the BRAVO diabetes microsimulation model, serves as Associate Editor of Value in Health and Co-Editor-in-Chief of Pharmacoeconomics and Policy, and received the 2025 Hartzema Distinguished Speaker Award.
